Notice of Privacy Practices

A spacious waiting room with empty chairs and sofas, large floor-to-ceiling windows, and warm sunlight streaming in, creating long shadows on the floor. The scene appears calm and quiet.

The Interventional Clinic (“the Clinic,” “we,” “us,” or “our”) is committed to protecting the privacy of your health information. We are required by law to maintain the privacy of your protected health information (PHI), to provide you with this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

How We May Use and Disclose Your Health Information

The following describes the ways we may use and disclose your protected health information. Not every use or disclosure will be listed, but every way we are permitted to use and disclose information will fall within one of the categories below.

For Treatment

We may use and disclose your health information to provide, coordinate, or manage your care and related services. This includes sharing information with physicians, nurses, technicians, referring providers, and other personnel who are involved in your treatment. For example, an interventional radiologist may share imaging results with your referring physician to coordinate your care.

For Payment

We may use and disclose your health information so that we can bill and collect payment for the services we provide. For example, we may share information with your health insurer to obtain prior authorization for a procedure or to determine coverage and benefits.

For Health Care Operations

We may use and disclose your health information to support the business activities of the Clinic. This includes quality assessment, staff training and review, accreditation, licensing, and general administrative activities that help us run the practice and improve the care we provide.

Appointment Reminders and Health-Related Information

We may contact you to remind you of an appointment, to tell you about treatment alternatives, or to share information about health-related benefits and services that may be of interest to you.

Individuals Involved in Your Care

Unless you object, we may share information with a family member, friend, or other person you identify as being involved in your care or payment for your care, limited to the information relevant to that person’s involvement. We may also disclose information to notify such a person of your location or general condition.

As Required by Law

We will disclose your health information when required to do so by federal, state, or local law.

Public Health and Safety

We may disclose your health information when necessary to prevent or lessen a serious threat to health or safety, and for public health activities such as reporting disease, injury, vital events, and adverse reactions to medications. We may also disclose information for purposes including health oversight activities, reporting suspected abuse or neglect, judicial and administrative proceedings, law enforcement purposes, coroners and medical examiners, organ donation, certain research activities, workers’ compensation, and military or national security purposes, as permitted by law.

Uses and Disclosures Requiring Your Authorization

Most uses and disclosures of psychotherapy notes (where applicable), uses and disclosures for marketing purposes, and any sale of protected health information require your written authorization. Other uses and disclosures not described in this notice will be made only with your written authorization. You may revoke an authorization in writing at any time, except to the extent we have already acted in reliance on it.
